Death Records
The City of Alton Clerk’s office issues death certificates for deaths that occurred in Alton, Godfrey, Moro, or Foster Townships dating back to the 1920’s.
If the death did NOT occur in Alton, Godfrey, Moro, or Foster Township, please contact the appropriate county or state vital records office. For deaths elsewhere in Madison County, contact Madison County Vital Records at (618) 692-6290.
Death records are confidential and are not considered public information under the Freedom of Information Act.
Genealogical copies of death records of persons who died more than 20 years ago. Please note out records only date back to 1920’s
Who May Obtain a Death Certificate
Under Illinois law, death certificates may only be issued to authorized individuals, including:
1. Immediate family members
2. Legal guardians or representatives.
3. The informant listed on the record
4. Individuals with a documented personal or property interest
5. Those presenting a court order
6. Funeral Directors
If you are requesting a death certificate as the duly authorized agent, legal representative, someone claiming a legal personal or property interest, please know that you must provide proof.
To request a death certificate online, you must provide a valid, non-expired photo ID. Accepted forms of identification include:
A valid, non-expired photo ID is required. Acceptable forms include:
• Driver’s License
• State Issued ID
• U.S. Passport
• Military ID
• FOID Card
• Conceal and Carry License (CCL)
Please note: Social Security Cards will NOT be accepted
Fees
• $23.00 for the first certified copy
• $8.00 for each additional copy of the same record ordered at the same time
• If mailed there is an additional fee of $1.50. Certificates will be mailed through USPS (first class mail) after processing.
Accepted payment methods include cash, check, money order, Visa, MasterCard, Discover, and American Express. Checks/money orders should be made payable to the City of Alton. Please note: Paying with a check online, will delay the process by 5 additional business days.
When copying identification documents, enlarge them to 115% of their original size for clarity.
